If a strategy is developed and positions placed automatically from the Strategy monitor, is the only way to tell what the starting position is by running a backtest and seeing what the open positions are?

Yes, if you need to see the state of the Positions, you'd have to run it in the Strategy Window. It will depend on the starting point too.
WL8 Build 6 (out tomorrow), we're doing more to check account positions to match exit signals. Already if you use Portfolio Sync preferences, an exit signal for a position you don't own should automatically not be placed with the broker.
