Auto-Stage Alerts intermittently not showing up

I am running a single coded (simple) strategy in Monitor. The strategy is activated and auto-stage is enabled. No strategy window is open. Alerts appear in Monitor but many do not make it to Orders. Roughly 10 alerts/hour, maybe 1 in 4 is staged to Orders. Any thoughts?
Answer from Fidelity: If an identical alert already exists in Orders the new alert will not be added to Orders.

That answer is correct if it applies to your situation. If it doesn't, you might be able to figure it out by answering these questions:

1. How many accounts do you have?
2. Which is your default account? (Preferences > Trading)
3. Did you associate accounts to the Strategies?
4. Which account are you viewing in the Orders tool?
